What's your diagnosis?

IVCe que montre l'IRMWhat the MRI shows

  • Right intraparotid ovoid, sharply marginated mass predominantly in the superficial lobe, exhibiting isosignal on T1-weighted imaging and heterogeneous signal on T2-weighted imaging, showing hyperintense areas consistent with myxoid areas.
  • En coronal T2 FS : capsule périphérique en hyposignal.
  • Coronal T2 FS reveals a peripheral low signal intensity capsule.
  • Diffusion et cartographie ADC : hypersignal diffusion, ADC élevé (ADC : 1,8 × 10⁻³ mm²/s).
  • Diffusion-weighted image and corresponding ADC map demonstrate an increased diffusivity with high ADC values (rADC: 1.8 × 10⁻³ mm²/s).
  • T1 FS après gadolinium : rehaussement hétérogène.
  • Heterogeneous enhancement on T1-weighted fat-suppressed MRI following gadolinium administration.
  • Progressive enhancement following dynamic gadolinium injection without wash-out, following a type A enhancement curve.

  • The most common benign parotid gland tumor in adults.
  • Le plus souvent dans le lobe superficiel.
  • Usually in the superficial lobe.
  • Hypersignal T2 très intense.
  • Very high T2 signal.
  • ADC values higher than other parotid tumors (1.3–2.2 × 10⁻³ mm²/s).

Risque de dégénérescence

Tendency to degenerate

  • Malignant transformation reported up to 15% if left untreated: carcinoma ex pleomorphic adenoma.
  • Signes de malignité : contours infiltrants, multicentricité, hyposignal T2, ADC < 1,2 (× 10⁻³ mm²/s).
  • Signs of malignancy: infiltrative margins, multicentricity, hypointense T2 signal, ADC value <1.2 (× 10⁻³ mm²/s).
